How the Referral Process Works

Submit a Referral
Contact our admissions team by phone, fax, or through this form. Provide basic patient demographics and a brief clinical summary.
Clinical Review
Our clinical team reviews the referral and conducts a pre-admission assessment — typically completed within 2 hours of receipt.
Authorization & Insurance
We handle insurance verification and prior authorization. Our team coordinates directly with payers to minimize delays.
Placement Confirmed
Once approved, we coordinate directly with the discharging facility to confirm transfer logistics and admission timing.
Eligibility
Admission Criteria
Terner Health accepts patients who meet the following general criteria. Our clinical team conducts an individualized assessment for each referral.
We Accept
- Post-acute patients requiring skilled nursing or rehabilitation
- Patients with complex wound care or IV therapy needs
- Individuals with dementia or cognitive impairment requiring a secure environment
- Patients requiring ventilator management or tracheostomy care
- Individuals with serious illness seeking palliative or comfort-focused care
- Patients requiring dialysis coordination and support
- Post-surgical patients needing intensive rehabilitation
Not Appropriate For
- Patients requiring acute hospital-level care
- Individuals who are medically unstable and require ICU monitoring
- Patients with active, uncontrolled psychiatric conditions requiring inpatient psychiatric care
Documentation
Required Documentation
To expedite the admission process, please have the following documentation available at the time of referral.
Face Sheet
Patient demographics, insurance information, and emergency contacts.
Physician Orders
Current physician orders including medications, diet, activity level, and any special precautions.
History & Physical
Most recent H&P, including primary diagnosis, relevant medical history, and current clinical status.
Nursing Assessment
Current nursing assessment including skin integrity, fall risk, and functional status.
Medication Reconciliation
Complete and current medication list with dosages and administration routes.
Lab & Diagnostic Results
Recent lab work, imaging reports, and any relevant diagnostic results.
